Wax-up

The first step towards smile designing involves dentists taking an impression of the patient’s teeth and making a set of study models. The changes are first performed on these models in wax, after which the dentist proceeds onto the next step. This process is called a wax-up.

3D Smile Designing

This step involves a digital treatment planning tool that offers a treatment plan which is wholly based on a thorough analysis of the patient’s dental and facial proportions. CBCT Case Planning and 3D Modelling

Dental Cone Beam Tomography [Dental CBCT] and CT methods are used to detect the extent of bone invasion in oral cancer, and are crucial for therapy planning and prognosis. CBCT imaging helps in the evaluation of the location, size, extent, and expansion of the lesion, along with the involvement of any surrounding vital structures. It is also useful in detecting osteolysis i.e. the extent of destruction of bone in patients with oral cancer. The images obtained from CBCT and CT imaging are sometimes turned into 3D models to assess patients better.

Computer Visualisation of the Final Result

CBCT imaging in tandem with appropriate software and virtual patent-specific models enables the examination of the lesion and surrounding hard and soft tissues. This is used to generate virtual models and assess post-surgical facial structures, analyse treatment requirements and devise an effective post-surgical plan.

Surgical Guides

Digital planning enables the surgeon to utilise a combination of supporting structures during the treatment planning and rehabilitation process. These structures can include teeth, mucosa, and surrounding hard tissue structures. Surgical guides are a great way to reduce errors, avoid peri- and post-operative complications, and increase the efficacy of the rehabilitation process.

Fabrication of Custom Implants

After restoring the jawbone through bone augmentation, dental implants need to be placed to restore the physical appearance and function of the mouth. This can be challenging in oral cancer patients, as the bone into which the dental implants are placed has often undergone radiation. Additionally, implant failure increases when implants are placed in previously treated bone because healing can take much longer. Success rates for dental implants after radiation therapy differ based on where in the body the patient’s cancer was treated. Imaging and 3D Modelling

Imaging systems such as Dental Cone Beam Tomography [Dental CBCT], CT and MRI are very useful in determining dental and jaw-related fractures, vertical root fractures, condylar fractures and multiple jaw fractures with bone displacement. The images obtained from these systems is helpful in assessing the changes in hard and soft tissues in and around trauma-affected regions, which are then turned into virtual models, in order to analyse treatment requirements and devise an effective post-surgical plan.

These structures can include teeth, mucosa, and surrounding hard tissue structures. Fabrication of Splints & Custom Implants

The final prostheses should replace not only missing teeth but also lost soft tissues and bone, and they should restore the hard palate, residual alveolar ridges, and, in some instances, the soft palate.
Splints and custom implants work in tandem to aid repair and prevent further damage to the patient. Splints are useful for holding the fractured/treated bone in place, and are also used in preventing further displacement of teeth and related jaw structures. On the other hand, fabrication of the right implants helps in recreation of complete maxillofacial structures, functions and esthetics, while preserving minimum thickness so as to reduce weight. We use the Dental Cone Beam Tomography [Dental CBCT] to evaluate the position of teeth, bone structures, joints and the airway. This technique helps us in estimating the accurate placement of implants, provides 3D evaluation and aids in complex root canal diagnosis. This computerised scan uses advanced technology to generate three-dimensional images, which we sometimes turn into 3D models to assess our patients better.

Provisional Restoration

Once we evaluate the nuances of our patient’s maxillofacial structures and functions, achieve appropriate tooth preps (if any) and place the implants in the patient’s jaw bones, it becomes essential for us to provide our patients with a provisional restoration which maintains the esthetic and functional qualities of natural teeth, while also helping prevent migration of the teeth near the implant sites and the over-extrusion of opposing teeth. Our provisional restorations are made to deliver maximum comfort to patients, while making sure they check all the boxes of necessary requirement.

Final Restoration

Once the dental implants are secured in place, adequate osseointegration has taken place, and the provisional restoration phase indicates maximum patient satisfaction, we proceed with the placement of the final restorations. Our final restorations are either screw-retained or cement-retained, and restore all aesthetics and functions of natural teeth.
